Training module
Formative assessment with exit tickets and error analysis
Formative assessment gamit ang exit tickets at error analysis
Design two-minute exit tickets, read the patterns in wrong answers, and decide what to reteach tomorrow, with help from the Assessment Center's auto-scored quizzes.
- Time
- 35 minutes
- For
- All SHS mathematics teachers
- Tools
- Presentation, Spreadsheet
You will be able to
- Write an exit ticket whose distractors each reveal a specific misconception.
- Run a quick error analysis by hand or with the Assessment Center report.
- Choose a reteaching move for the most common error.

What formative assessment is for
Formative assessment changes tomorrow's lesson; it is not graded. The ILAW format asks for it in every session and requires accommodations. The quickest reliable form is a two- or three-item exit ticket collected in the last five minutes.
Designing diagnostic distractors
ExplainEach wrong choice should come from a known error, so that the choice a learner picks tells you what they think. For f(x) = x² − 3 at x = −4: the correct answer is 13; the distractor −19 comes from writing −4² = −16; the distractor 19 from dropping the sign on −3. The problem bank on this site stores a misconception note per distractor for exactly this purpose.
Three exit-ticket formats
- Which student is correct? Show two worked solutions, one with a common error; learners pick and justify.
- One problem, one sentence: solve, then write what you found hardest.
- Confidence check: solve, then rate confidence 1–3. Low confidence with a correct answer still needs attention.
Error analysis in five minutes
EvaluateSort tickets into piles by the error, not by right/wrong. Count the piles. If one error has more than a third of the class, reteach it to everyone tomorrow; if it is a small group, plan a guided-practice station. The Assessment Center does this sorting automatically for auto-scored quizzes and shows which misconception each distractor signals.
Reteaching moves
| Error pattern | Move |
|---|---|
| Sign errors | Number line with coloured arrows; learners narrate each step aloud. |
| Procedure applied in wrong situation | Compare-and-contrast pair of problems side by side. |
| Vocabulary confusion | Frayer model for the term, in English and Filipino. |
| Reads the problem wrong | Highlight the question, restate it in own words before solving. |
Try it
- Write a three-item exit ticket for tomorrow's lesson with a misconception note for each distractor.
- Run one quiz from the Assessment Center with a class and read the error-analysis report.
- Pick the top error and write the reteaching move you will use.
